From there, we stay with you, making sure nothing falls through the cracks through ongoing monitoring, testing, diagnosis, and treatment.
Begin your care
Tell us about your health history, your family history, your goals, and provide any notes or research you may have compiled. We guide you through onboarding, create your chart, and begin the process of gathering your records. If you already have records, you can upload them too.
We gather your records
We retrieve your medical records across providers, hospitals, labs, and imaging centers. Electronic records can begin arriving quickly. Additional time may be required to obtain older, non-network, or paper-based records.
We organize your full history
Your history is brought together in one secure, searchable place and updated as new information arrives - so you and your care team can work from a clearer, more complete picture over time.
A physician reviews the full picture
Our physicians review your record, identify important patterns and gaps, and help clarify next steps. When appropriate, Modulus physicians can order testing, diagnose, treat, and coordinate referrals to help move care forward.
We stay with you
Your care does not stop after the first review. Modulus monitors your health daily, updates your chart with every visit, tracks symptoms over time, and steps in when something needs attention. A physician is always available when you need one.
